Blood Coverage Claims

Blood Assurance

1. After the patient has been discharged from the hospital, call The Blood Center’s Patient Claim Representative at 1-800-86-BLOOD to request a claim packet.

2. Upon receipt of the packet please complete all information requested on the Blood Assurance Claim Form. Missing information may delay the process of the claim.

3. Mail the completed form with any requested supporting documentation to:

4. The Blood Center will review the form and then send a request to the hospital for verification of blood component usage.

5. Once the confirmation for usage is received from the hospital, which usually takes approximately 30 days, the payment will be processed. Payments will be made directly to the patient and will be generated on a monthly basis.

6. Claims must be submitted within 6 months from the date of blood component usage.

7. A separate claim form should be submitted for each hospital stay.

Blood Replacement

1. Call The Blood Center’s Patient Claim Representative at 1-800-86-BLOOD to request a claim packet.

2. Upon receipt of the packet please complete all information requested on the Blood Replacement Claim Form. Any information left blank may delay the process of the claim.

3. Mail the completed form with any requested supporting documentation to:

4. The Blood Center will review the form, then process the claim for payment. Payments will be made payable directly to the patient and will be generated on a monthly basis.

5. Claims must be submitted within 6 months from the date of replacement donation.
